2. The primary contact for all publishing correspondence.
3. The name of each individual or entity that will receive a portion of the royalties generated from the sale of your published book; include their relationship to the book; i.e. agent, co-author, etc.
4. An author biography to be included in your book and given to book wholesalers and retailers.
